APC offers a variety of services for children with life-sustaining
medical conditions who require technical medical equipment
and/or treatments in order to thrive at home. The APC team
includes professionals experienced in pediatric nursing, communication
disorders, assistive/adaptive equipment, case management,
independent living, vocational services, transition counseling
and when needed grief counseling.
Our Pediatric Nurses specialize in caring for children
and we never forget that we are a guest in each family's home.
Sometimes this special care must follow the child to school
- so we go back to the classroom! We have even been seen at
special summer camps with our kids. Our nurses provide a full
range of pediatric services which include but are not limited
to:
- Tracheostomy care
- Respiratory Treatments
- Seizure Monitoring and Assessment
- Ventilator Care
- Wound Care
- Gastrostomy Tube Care/Feedings
- IV Therapy
- Client/Family Education and Training
Skilled Nursing care is often paid for by private insurance,
private or self-pay, Medicaid Comprehensive Care Program (CCP),
Medically Dependent Children's Program (MDCP) and other third
party payors.
Our Rehabilitation Team provides information about
options and choices for the future.
Medical Social Worker: The agency is committed
to the health and care of the clients and staff and employs
a full-time Licensed Master Social Worker (LMSW) who specializes
in the needs of infants, children, teens and their families
with medical/disability issues. Grief counseling, emotional
and financial counseling with the client and their families,
assistance in resources and referrals as well as case management
are available. Counseling for the staff who deal with these
issues daily is also available as a support to the treatment
team.
